x.x.42.x (third octet will always be the store number...10.3.42.x)

x.x.x.5 (fourth octet for the NVR will always be designated at .5...10.3.42.5)

The Gateway is always .126

The Primary DNS is always the same as the Gateway. Nothing in secondary, leave blank.

The ports should always be:

RTSP: 64999

Server: 65000
